As a result of the reasons described herein below, I could not comply with the prescribed rules applicable to keep my Dedicated Sport Shooter Status in place and hereby request condonation for such non-compliance.

I hereby confirm that the information contained herein is true and correct. Any false information may lead to my immediate expulsion from __ (Name of Association) and will be reported to the Central Firearm Registry for further investigation by the SAPS.
